When you first visit one of our locations, we need to get a bit of information such as your name, date of birth and your insurance in order to provide you with care. Download and fill out our New Patient Registration to supply us with required information. You may be able to fax or mail the information in prior to your first appointment. Verify with your provider before sending.
In order to provide everyone with good care, we have a few rules that all our clinics abide by. Summaries of relevant policies are below. You may also want to check with your clinic for any clinic-specific policies.
We expect all patients to keep their appointments and to arrive prior to the appointment time as the clinic has requested. We understand there are times when you cannot make your scheduled appointment, please let us know as soon as possible. When you first establish care with a clinic, you should receive a copy of this policy.
The policy in brief:
The primary goal of MSU HealthTeam is to ensure that every patient receives proper care. To achieve this, a healthy provider-patient relationship is required. Therefore, there are times when we cannot continue to provide a patient with the appropriate level of care and may discharge that patient from care.
It is the policy of the MSU HealthTeam to ensure the continuity of quality care and the sustenance of the mental and physical health of everyone on MSU Campus. Disruptive, non-compliant, or threatening behavior to a patient’s own care, care of other patients, visitors, staff and general operations of MSU HealthTeam, can result in discharge of care from a provider, clinic, MSU HealthTeam and from MSU Campus.
