Disease Registry Software Tool on the way to MSU HealthTeam
MSU HealthTeam and Health Information Technology (HIT) have begun a project to implement a disease registry system called CareManager. This tool provides timely information about a patients’ health status to providers and support staff for patients diagnosed with, and at-risk of, chronic disease. Forms integrated within the EMR display best practice guidelines and the patient’s health status for chronic measures at the point of care. A patient population tab displays the status for all patients, which is used by support staff for follow up activities. CareManager utilizes health data stored in our electronic health record, to evaluate key health indicators against evidence-based treatment guidelines. This approach informs decision making at the point-of-care and allows providers to easily monitor process compliance and patient goal-attainment for a range of disease states.
Currently, there are nine registry modules available (Asthma, Diabetes, Stroke Prevention, Coronary Heart Disease, Tobacco Cessation, Cancer Screening, Heart Failure, Osteoporosis, and Drug Safety). HIT, EMR and Quality are working with a task force of physicians, nurses, support staff and administrators to develop workflows and make recommendations about implementation strategies and use to the Clinical Informatics Steering Committee, (CISC) and Quality Council. Consensus was reached at the June CISC and Quality Council meetings to implement the Asthma and Diabetes modules in two primary care clinics (COM Family Medicine and CHM Pediatrics) as part of the first phase, starting in August 2009. In the second phase, additional clinics will implement these modules, and then additional modules will be implemented over time for both primary and specialty practices where applicable.
The CareManager disease registry will benefit not only patients by facilitating guideline concordant care, but also providers and the HealthTeam as we now qualify for additional incentive monies.
